1. Who can refer to the Hospital?

Systemic rehabilitation in a day centre/department is referred by:
• physician of the following clinics: rehabilitation, trauma and orthopaedic, neurological, rheumatology, postural disorder, paediatric endocrinology, paediatric diabetology
• GP (in the case of exacerbations of chronic diseases)
• physician of the following departments: trauma and orthopaedic, surgical, neurosurgical, neurological, rheumatology, internal medicine, oncology, urological, gynaecology, paediatric, paediatric endocrinology, paediatric diabetology, systemic rehabilitation, neurological rehabilitation, pulmonary rehabilitation, cardiological rehabilitation

Rehabilitation of children with developmental age disorders in a day centre/department is referred by the physicians of the following hospital departments and specialist clinics:
• neonatal,
• rehabilitation,
• orthopaedics and traumatology of the locomotor system,
• neurological,
• rheumatology,
• paediatric surgery,
• paediatric endocrinology
• paediatric diabetology
• GP.

Referral for systemic rehabilitation in stationary conditions is issued by the physician:
• of the following departments: trauma and orthopaedic, surgical, neurosurgical, neurological, rheumatology, internal medicine, oncology, gynaecology, urogical, paediatric, cardiology
• in the case of exacerbations of chronic diseases, physician of the following clinics: rehabilitation, trauma and orthopaedic, neurological, rheumatology

Referral for neurological rehabilitation is issued by the physician of:
• the following clinics: neurological, neonatal, neurosurgical, paediatric, trauma and orthopaedic, rehabilitation, paediatric surgery
• of the following departments: anaesthesiology and intensive therapy, neonatal, neurology and neurosurgery, orthopaedics and traumatology, paediatric surgery, infectious diseases, paediatric

Referral to pulmonary rehabilitation department is issued by the physician of:
• the following clinics: tuberculosis and lung diseases, rehabilitation, allergology, thoracic surgery
• of the following departments: pulmonary, tuberculosis and lung diseases, thoracic surgery, cardiology, laryngology, internal medicine, allergology, oncology and paediatrics (in the case of pulmonary rehabilitation of developmental age). Referral for hospital treatment of : allergic diseases and respiratory system disorders is issued by the health insurance physician or physician, who is not a health insurance physician (e.g. a physician treating without a contract with the National Health Fund, so-called "privately").

2. How long do I have to wait for the treatment/rehabilitation in the Hospital?

The waiting time depends on the department, to which a child would be qualified.

3. How long will I stay in the Hospital?

The length of a Hospital stay depends on the disease, health condition and patient’s individual needs. The length of stay is initially projected on admission to the Hospital, after a medical examination conducted by the attending physician, it may be subject to change.

4. Can I arrange the date of stay in the Hospital by phone?

Yes, after the contact with the department, to which a child has been qualified.

5. Is there a school and a nursery at the Hospital?

The primary and lower-secondary school-aged children attend the classes in the hospital School. The children of the pre-school age remain under the care of nursery practicioners, and the upper-secondary youth – in the education groups.

6. Can I choose the date of admittance?

The hospital stays are planned according to the waiting list.

7. Is there a shop/cafeteria near the Hospital?

There are no shop or cafeteria facilities in the Hospital, but there are two sweets and drinks and juices vending machines as well as coffee/tea vending machines.

8. Is the parking lot at the Hospital chargeable?

The parking lot is free of charge.

9. Can a child after the age of 18 be admitted to the Hospital?

The stay of patient of legal age is possible upon obtaining the consent of the National Health Fund. A parent in person or a hospital representative on the request of a parent may apply for the consent of the National Health Fund.

10. How long is the referral to the clinic valid for?

The referral to the specialist clinic (excluding the rehabilitation one) is valid until the treatment of the disease indicated in the referral of the referring physician is accomplished. The referral to the rehabilitation clinic is valid for one year from the date of the first visit.

11. What is the price for a child’s carer of staying in the Unit of 24-hour Care over the Children with Carers?

•room with bath + full board - 50 zl /day
• room without bath (with wash basin) + full board - 41 zl/ day
• room without bath (without wash basin) day
There is a possibility to opt-out of full board. The price of full board is 15 zl ( breakfast and supper 4 zl each, dinner 7 zl) + full board - 32 zl/day

12. Can the children that are not the patients of this Hospital stay with the parent in the Unit of 24-hour Care over the Children with Carers?

NO.

13. In the case of the Unit of 24-hour Care over the Children with Carers, is it possible, during the hospital stay, to go home for a weekend?

NO.

14. In the case of the Unit of 24-hour Care over the Children with Carers, when there are two carers does the room price change?

Yes, the price for the second carer remains the same as for the first one.

15. Is there a wireless internet facility?

Yes.

16. In the case of the Unit of 24-hour Care over the Children with Carers, can another person that is not the child’s parent take care of the child during his/her hospital stay?

Yes, but on the date of admittance and discharge the presence of one parent is required.

17. What kind of medical documentation should be brought to the hospital?

• hospital discharge summary reports
• X-ray images
• current medical examination results
• disability certificate,
• opinion of the psychological and pedagogical counselling centre – if it is available.
